Please fill out the form below in its entirety, sign and submit. Please arrive on the northeast side of the Brent's Pharmacy building and park in the space labeled "Covid Testing Only" at your scheduled time. Do not leave your vehicle. We will meet you curbside. Just as a reminder, the cost of the screen is $75. The test is HSA/FSA eligible. Results will be sent to you within 30 minutes of completion.

Results are available 20 minutes after testing. You will be notified by text or phone call with results within 30 minutes and an official report will be emailed to you by the end of the day.

Cough, fever (>101), shortness of breath (difficulty breathing), fatigue, runny/stuffy nose, muscle pain/body aches, headache, diarrhea. Loss of taste and/or smell is unique to a Covid infection.
